These Terms of Service govern your use of the Wellness House NYC website and clinical services. By accessing this website or receiving treatment at Wellness House NYC, you agree to these terms in full.

Services Provided
Wellness House NYC provides peptide therapy, IV therapy, vitamin injections, and related longevity and wellness services at its clinic located at 140 West 58th Street, Suite 1A, New York, NY 10019.
All treatments are physician-supervised. Dr. Lanna Cheuck, DO, FACS, serves as Medical Director and oversees all clinical protocols.
Services are available to patients who meet clinical eligibility criteria as determined during consultation. Wellness House NYC reserves the right to decline treatment when clinical judgment indicates it is not appropriate.
Patient Responsibilities
Patients must provide accurate and complete medical history, medication lists, and health information during intake and at each subsequent visit.
Patients must follow prescribed treatment protocols, attend scheduled follow-up appointments, and complete required lab work at the intervals specified by the treating physician.
Failure to disclose relevant medical information may affect treatment safety and outcomes. Wellness House NYC is not responsible for complications arising from incomplete or inaccurate patient disclosures.
